Supporting safe, confident hospital discharge with the right support in place.
We’re looking for motivated and dynamic Social Worker to join Torfaen’s IAA Team within Adults & Communities on a fixed-term contract, focussing on hospital discharge
You’ll play a key role at a critical time, supporting people to return home with the right support in place, with a strong focus on what matters, strengths, and independence. Working as part of a multi-disciplinary team, you’ll build strong relationships with individuals, families, and partners across hospital and community settings, ensuring timely, person-centred decisions and positive outcomes.
We are a multi-disciplinary group of teams who work closely with partner agencies to meet the social care needs of adults aged 18 and over. Our aim is to work proactively with individuals to promote choice, control, independence, and positive risk-taking, enabling people to achieve outcomes in the areas that matter most to them, particularly at key transition points such as hospital discharge.
The successful candidate will be a registered Social Worker. You will be committed to working in a strengths-based way, focusing on “what matters” to individuals and supporting timely, person-centred discharge planning.
We are looking for someone who demonstrates flexibility, enthusiasm, and dedication, and who can work positively across both hospital and community settings. You will build and maintain strong, meaningful relationships with individuals, families, and carers, ensuring they are supported to understand their options and achieve outcomes that are important to them.
