Purpose of the post
To work as part of a residential team providing short breaks for disabled children:
•Ensure that the people of Gwynedd are at the heart of everything we do.
•As a member of the team, on a day to day basis provide direct care and preventative interventions for disabled children and young people as well as other supporting tasks within the unit.
•Work closely with internal and external partners to achieve the best outcomes for service users.
•Provide a range of interventions, utilising the most effective and purposeful intervention to meet the needs of children and their families.
Responsibility for functions
Equipment – Kitchen/cooking, ensuring appropriate use of the unit’s facilities ( including hoists).
Main duties
•To contribute to developing care and support plans, taking part in reviews and other meetings, and to assist in putting plans into action.
•To be part of a team, supporting co-workers and receiving support as necessary.
•Develop good working relationships with families to develop trust, improve contact and access to services and assisting with communication and co-ordination.
•Develop families resilience and confidence in order to empower them to make informed decisions and take responsibility by developing their skills.
•Attend planning and review meetings and contribute to the development of action plans. Keep up to date case notes and notes of meetings attended. Provide summaries and reports in relation to individual case work.
•Encourage and support families to follow the action plan.
•Plan and provide a range of interventions to support families in their local community.
•Encourage and enable children and their families to live independently in the community.
•Encourage and enable children to reach their full potential, in a way that promotes their health, development, safety and welfare.
•Ensure that the service meets the needs of the child, following any individual behaviour plans as needed.
•Assist and support families to provide continuity of care for children
•Be vigilant against signs of unhappiness and abuse, and ensure that children are safeguarded and monitored.
•Inform the line manager, or other suitable person, of any poor practice or concerns or evidence which might suggest this to be the case.
•To take care of children’s physical needs as necessary eg. cooking, washing, ironing, shopping, managing money or assisting children to undertake these tasks.
•Pass on relevant information regarding events/ incidents eg at changeover of shift.
•Talk to and listen to children, recording relevant information.
•Attend team meetings as well as any other meetings deemed appropriate by the team manager.
•Receive regular supervision.
•Be flexible in order to ensure appropriate staffing levels within the unit.
•Responsibility for self-development.
•Ensure compliance with Health and Safety rules in the workplace in accordance with the responsibilities noted in the Health and Safety at Work Act 1974 and the Council’s Health and Safety Policy.
•Operate within the Council’s policies in relation to equal opportunities and equality.
•Responsible for managing information in accordance with the Council’s information management guidelines. Ensure that personal information is treated in accordance with Data Protection legislation.
•Commitment to reducing the Council’s carbon emissions in accordance with the Carbon Management Plan, and to encourage others to act positively towards reducing the Council’s Carbon Footprint.
•Undertake any other reasonable duty which corresponds to the salary level and responsibility level of the job.
•Responsibility to report any worry or suspicion that a child or vulnerable adult is being abused
Special circumstances
The post involves working on a rota basis outside usual office hours in order to provide an overnight service, including weekends and sleeping in duties.
The post is located in Penrhyndeudraeth.
