We are currently looking for a highly motivated, solutions focused person with an enthusiastic enabling attitude to work as Practice Coordinator within the Torfaen Integrated Reablement Service for a fixed term period until 31st August 2027 .
The Integrated Reablement Service is multifunctional & provides short-term support and guidance for people living in Torfaen to maintain and improve independence in their own homes or within Ty Glas y Dorlan. The service is flexible and responsive and provides practical help, personal care and support in a way that is focused on the individual and is based on ‘what matters’ to them.
The Practice Coordinator supports the team to provide services to aid individuals live independently, safely and well in their own homes. The Practice Coordinator should work in a person-centred outcome focused manner, working with other social care practitioners to embed practice that is strengths based, focused on promoting independence, skill building, positive risk taking social inclusion and community connection. The Practice Coordinator must be able to demonstrate excellent risk management and manual handling skills.
This Integrated Reablement Service operates between 07:00 am-23:00 pm, 7 days a week including bank holidays and the role will require weekend working on a rota basis in line with Torfaen Councils Domiciliary Care Service.
