Job description

To support young people to achieve stability and permanence in their housing. To promote the wellbeing of young people working towards independent living by increasing their resilience and supporting them to work towards personal goals. To support Social Workers in case management responsibilities and provide a service to young people with housing need. The post undertakes pre-tenancy work and skills building for independent living with individuals to build their capacity to take on a tenancy or right to occupy a home after their 16th birthday. This post is grant funded via Housing Support funding, so support work must focus on tasks within these remits.

Responsibilities

Support young people aged 16+ to develop independent living skills
Provide practical and emotional support to promote wellbeing, resilience, and positive life choices.
Undertake pre-tenancy work and prepare young people for maintaining a tenancy or independent accommodation.
Work alongside Social Workers and other professionals to support care planning, assessments, and reviews.
Build positive, trusting relationships with young people to encourage engagement and participation in their plans.
Contribute to assessments by gathering information and supporting young people to express their views and needs.
Support the implementation and monitoring of care or pathway plans, reporting on progress and outcomes.
Liaise and collaborate with partner agencies (e.g. housing, education, health services) to ensure coordinated support.
Promote safety and wellbeing by identifying risks, responding to safeguarding concerns, and following procedures.
Deliver outreach support, including home visits and accompanying young people to appointments where appropriate.
Maintain accurate, timely records and documentation in line with service requirements.
Encourage young people to build support networks and access community resources to sustain independence.

Focus on grant-funded projects, adhering to Housing Support funding guidelines, to provide sustainable and effective support.
Engage in community outreach initiatives to identify housing opportunities, build a strong support network, and create a sense of community for vulnerable youth.
Maintain accurate and confidential records of all support provided, ensuring data privacy and security.
Stay updated on industry trends and best practices through regular team meetings and training sessions, fostering continuous professional development.
Work independently, manage caseloads efficiently, and prioritize tasks to provide timely and tailored support to each young person.
Promote a positive and supportive environment, encouraging young individuals to reach their full potential and achieve their personal goals.

A relevant qualification in Social Work, Youth Work, or a related field is preferred, demonstrating a strong commitment to youth development and support.
Experience in a similar role or working directly with young people is highly desirable, offering valuable insights and a deep understanding of their unique needs.
In-depth knowledge of housing policies, procedures, and support services is essential for effective guidance and advocacy.
Excellent communication skills, with a compassionate and patient approach, to build trust and rapport with young individuals and their families.
Ability to work independently, manage multiple tasks, and prioritize effectively, ensuring a tailored and efficient support system.
Proficiency in record-keeping software and data management systems to maintain accurate and organized documentation.
A valid driving license is advantageous for community engagement activities and outreach programs, enhancing your ability to support vulnerable youth.
