Job description
Do you want to help us make a difference by supporting people to live well at home? Then this could be the role for you! Collaboration is at the heart of this role, you’ll work alongside a range of teams in Swansea Adult Services including homecare, early help, therapies, social work, and commissioning teams, to make sure people receive the right support at the right time.
The focus of this office-based role is to support teams with the administration and coordination of timely and effective support for individuals. By facilitating early intervention and utilising reablement services, you will help prevent escalation of need and avoid unnecessary hospital admissions, enabling people to remain safely and independently at home. Through proactive support for professionals and efficient case administration, you will be crucial in supporting colleagues to help individuals regain confidence and independence, reducing reliance on acute services. Your focus on timely interventions and robust administrative processes will maximise the reach and impact of reablement and support services across Swansea, enabling more people to benefit from effective, person-centred care.
