Work base: Abergele, Colwyn Bay
We are looking for an enthusiastic and motivated Health & Social Care Support Worker to join our established Reablement Team. This is a rewarding role supporting adults (18+) in their own homes and community settings to regain independence, improve wellbeing, and achieve their personal outcomes.
As part of a multi disciplinary team—including Nursing, Physiotherapy, Occupational Therapy, Social Work, Community Equipment, Telecare and voluntary services—you will deliver a mix of social, health, and rehabilitative care as outlined in Multi Disciplinary Team care plans.
Key Responsibilities
• Support individuals at home to develop confidence, personal care skills and daily living activities, promoting maximum independence.
• Carry out prescribed clinical tasks such as physiological measurements (BP, pulse, temperature), urine testing, blood sampling, and oral hygiene (training provided).
• Record and report observations accurately in line with organisational requirements.
• Work flexibly as part of a service delivering 365 days per year.
Essential Criteria
• Level 2 in Health & Social Care and willingness to complete Level 3 within 12 months.
• Experience in health or social care settings, preferably in the community.
• Ability to follow guidelines, carry out delegated duties, and respond positively to feedback.
• Good general education.
• Ability to converse in Welsh with older people about their health and wellbeing.
• Ability to write in English (Welsh writing desirable).
