Work base: Disability Domiciliary Support Services

This will be achieved by putting the child / young person at the centre of all support planning and developing strategies that will enable the child/young person to develop skills in all aspects of their life to give them independence and feeling included in their communities based on what is important to them.
We are looking to appoint a Community Support Coordinator within our Disability Resources Team. The role of the Community Support Coordinator will be to support the registered manager in the delivery of services to individuals who have disabilities whilst ensuring the service remains compliant with the Regulation and Inspection of Social Care (Wales) Act 2016.
The current position will be supporting the registered manager to deliver support to individuals who have disabilities living in their own homes.
You will be expected to: • Co-ordinate the day-to-day deployment of staff: this will include accepting new referrals, allocating work and updating records. • Communicate with service users their families, social workers and other professionals on a day-to-day basis as required. • Provide supervision, support and guidance in a sensitive manner whilst ensuring that staff work safely and meet the quality set out by The Care Inspectorate for Wales. • Complete service delivery plans, which meet the identified outcomes for individuals. • Carry out risk assessments and manual handling assessments in line with Corporate Policy and Procedures. • Have completed an NVQ Level 2 in Health and Social Care or relevant subject. • Have the ability to achieve a QCF Level 3 in Health and Social Care for Children and Young People or Adults within a 3-year period following appointment.
What we are looking for from you: • Have the confidence and ability to solve problems and make decisions, which will affect service delivery and health and safety. • The ability to communicate on all levels with staff, individuals, other colleagues, other professionals, management, family and friends. • To be able to communicate in a friendly and caring way. • To be physical and emotionally resilient to deal with the demands of the role. • Be able to manage and prioritise your own workload.
Appointment to this post will be subject to 2 satisfactory references and Enhanced DBS checks.
This role offers flexible working options for a work life balance. This can include adjusting your working day and hybrid working, ie a balance of office and home working.
