Grade 7 | Full Time PermanentAre you passionate about supporting adults to live independently and achieve the outcomes that matter to them? Swansea Council is seeking a dedicated Social Services Practitioner to join our Adult Services team.
Working within a multidisciplinary team, you will undertake non-complex assessments and reviews, support care and support planning, promote wellbeing and independence, and contribute to safeguarding activity. You will work closely with individuals, carers, families and partner organisations to deliver person-centred, strengths-based support.
About You
We are looking for someone who has:
- A Social Care qualification at Level 4 or above or willing to work towards the Social Services Practitioner qualification
- Experience of working in a social care setting or related field.
- Knowledge of the Social Services and Well-being (Wales) Act.
- Strong communication, report-writing and IT skills.
- An understanding of safeguarding, confidentiality and data protection requirements.
- The ability to build positive working relationships with individuals, carers and professionals.
