Working hours: 37 Hours (usually 7 hours 24 mins per day Monday- Friday between 8:30 am and 5:00pm with 4:30 pm finish on Friday)

This is an exciting opportunity to join Team Caerphilly and provide support across the wider Community Resource Team.
We pay an attractive salary of £40,221 - £43,421 and offer access to training and development opportunities.We are seeking an individual who is experienced within hospital discharge, with knowledge of collaborative communication and how to be effective when completing Integrated assessments, MCA and considering CHC etc. to join Caerphilly's Community Resource Team.
To succeed in this role, you must be a reflective practitioner who is able to offer support to other team members when working with individuals to achieve their personal outcomes.
You should also be able to support student placements and the career development of others within the Community Resource Team. Enthusiasm, flexibility, and a desire to help people do what matters to them is a must.
For the role, we ask that you have: - A Diploma / Degree in Social Work or predecessor qualification, with successful completion of the Consolidation of Practice Programme within the first three years of practice (if qualified after 1 April 2016)
- Knowledge of the current social care legislative framework and national drivers within Adult Social care
- Substantial post qualifying experience within a social services environment
- A full UK Category B (Cars) driving licence and the use of a motor vehicle insured for business purposes to travel throughout the borough to visit clients and attend meetings.
