At Age Connects North Wales Central, we have opportunities available for Practitioners to join our growing team, supporting adults across Denbighshire and Conwy. We are not a care agency therefore personal care is not provided. Our Organisation We are an empowering charity that has been providing various community and residential services to people in North Wales for 30 years. We pride ourselves on the way we work with empathy and professionalism and have an established track record of managing projects and services effectively and efficiently, focusing on achieving positive outcomes for the people who utilise our services. The Role – Support Practitioner We are in the process of growing the reach and presence of Home Life, a chargeable service which is now in its 5th year of trading to assist residents across Denbighshire and Conwy. As a Support Practitioner, you will join a friendly and growing team of 9 comprising support workers, an Administrator and Manager, where you will typically find yourself providing to adults over 50: • Shopping (food, clothes, homeware) • Prescription collection • Attend appointments • Companionship (home and in hospital) • Cooking and preparing meals including light meals/snacks • Washing and ironing • Light house cleaning • Accompanying on trips out This list is not exhaustive, and requirements are tailored to each individual client. Our Requirements – Support Practitioner We are looking for empathetic individuals whose values align with our own, who genuinely care about the wellbeing of others. Therefore, we are not asking for you to have proven experience of giving support to adults (although this would be advantageous), but instead we ask that you have: • A desire to make a difference to the lives of those who live in our communities • Full UK driving licence and access to your own transport to travel around your designated area as required Although not essential, the following would be beneficial: • Experience of providing support to adults • Qualified or studying towards Level 3 in Health and Social Care or equivalent Due to the nature of the work undertaken by the charity, please note that successful applicants will be subject to a satisfactory DBS Check. The Package As a Support Practitioner, you’ll be employed on a 12-month contract, and will benefit from: • Pay rate @ National Minimum Wage (increasing in April 2025) + travel time + holiday pay, + mileage paid at 35p a mile • Flexible working hours to suit your current commitments
