ACCOUNTABLE TO: Director of Care

REPORTS TO: Team Manager

RESPONSIBLE FOR:

Supervision and support of Care Team, Senior Support Worker required within the Domiciliary Care aspect of our service to work with the team at Yr Hen Ysgol and Outreach Dolgellau (Locations in Gwynedd and Powys as the service develops).

The Senior Support Worker will assist the Team Manager in the management of the domiciliary care service within Yr Hen Ysgol and the Outreach services.

The post holder will be expected to demonstrate an understanding of the Aims and

Objectives of the organization and assist the Team Manager to maintain a quality service.

The post holder is responsible for the implementation and evaluation of evidence-based care under the supervision of the Team Manager and the Director of Care.

Applicants must have the following:

*Minimum qualification of NVQ/QCF Level 3 in Health and Social Care and/or working towards QCF Level 5 in Health and Social Care.

*Must be experienced in working with people who have complex needs.

*Must be able to support the team manager in leading/develop teams effectively and provide support, supervision and training.

*Must have a pro-active approach to each individual’s recovery and engagement in community living.

*Must be committed to contributing to the development and promotion of all individualized care packages.

*Must be a driver

*Is required to take on-call duties as part of the teams on call rota as directed by the Team Manager.

*Must understand the principles of managing staff rotas to meet the needs of the service users/contractual requirements.

*Must be able to address issues that arise with staff practice/conduct.
