Vale Community Resource Team are an Integrated service run by Vale of Glamorgan Council and Cardiff &The Vale UHB. We provide short term support and/or therapy to people living in their own home when they are first discharged from hospital or to prevent a hospital admission. Our aim is to maximise a person's independence.

About the role
Pay Details: Grade 4, SCP 5-7, £24,790 - £25,584 pro rata, £12.85 - £13.26 per hour plus enhancements

Hours of Work: Various hours available to cover our service up to a maximum of 30 hours per week

Main Place of Work: Various locations across the Vale

An exciting opportunity has arisen to join our integrated multidisciplinary team. As one of our Reablement Support Workers you will support people in their own homes with daily living tasks such as personal care and meal preparation.
