You will be part of a dedicated Intermediate Care Team within Carmarthenshire County Council, working closely with nurses, therapists, and other professionals to deliver short-term, person-centred care that makes a genuine impact on people’s recovery and wellbeing.
What you will do:
- Provide timely care and support during crisis calls, responding to urgent situations where people’s health or independence is at risk.
- Support individuals to regain skills and confidence in daily living activities after illness or hospital discharge.
- Deliver personal care (e.g. helping people to wash, dress, or use the toilet) with dignity and respect.
- Assist with meal preparation and medication to promote health and recovery.
- Encourage and enable people to work towards their rehabilitation and reablement goals, helping them return to independence.
- Monitor and record progress, reporting changes in condition and liaising with the wider health and social care team.
