Dementia Health & Social Care Support Worker (Job Share)
Working Pattern: Thursdays & Fridays
We are seeking to recruit an enthusiastic and motivated individual to join our Colwyn Bay Locality Resource Team as a Dementia Health & Social Care Support Worker on a job share basis.
This is a rewarding opportunity to contribute to a developing multi-disciplinary approach, supporting people living with dementia to remain independent and live well within their own homes.
As part of a multi-disciplinary team, you will work closely with health and social care professionals to provide coordinated, person-centred support. You will play a key role in helping individuals and their carers navigate health and social care services, ensuring they receive the right support at the right time.
Your duties will include supporting individuals in their own homes and community settings to maximise independence, promote wellbeing, and maintain quality of life. You will also act as a key point of contact for families and carers.
Key Responsibilities
* Deliver support in line with multi-disciplinary care plans, incorporating health, social care, and rehabilitative interventions
* Promote independence, confidence, and wellbeing for individuals living with dementia
* Support individuals and carers to access community resources and preventative services
* Undertake delegated clinical tasks such as:
* Monitoring vital signs (e.g. blood pressure, pulse, temperature)
* Collecting and testing urine samples
* Supporting oral health care
* Recording and reporting outcomes in line with organisational requirements
* Work collaboratively with professionals including nurses, physiotherapists, occupational therapists, social workers, and voluntary sector partners
You will be part of an established reablement and locality team, working across agencies including Community Equipment Services, Telecare, and voluntary organisations. The service focuses on enabling individuals to regain and maintain independence while strengthening community connections.
Essential Criteria
* Level 2 qualification in Health & Social Care, with a willingness to complete Level 3 within 12 months
* Experience working in a health or social care setting (community experience desirable)
* Ability to follow guidance, carry out delegated tasks, and respond positively to feedback
* Good general education
* Ability to communicate in Welsh with older people about their health and wellbeing
* Ability to write in English (Welsh writing skills desirable)
