This is a temporary, part time position for 21 hours per week (3 days a week) for an initial period of 3 months with a view to this being extended on a rolling basis. The Support Workers will implement care plans which will be outcome focused and aimed at promoting independence by helping individuals to do things for themselves, to maintain their ability to live life as independently as possible by supporting the whole person – addressing their physical, social and emotional needs. This may include components of personal care.

These roles are based within the Reablement and Interim Support Team. However once restrictions are relaxed due to COVID, you may be deployed to provide care and support in other services such as Extra Care.

Community Wellbeing Team Support Workers

22 hours per week on a rota basis

Community Wellbeing Team

 

The Community Wellbeing Team is a multidisciplinary integrated service that works closely with the ABMU.  The team provides a Reablement service as well as short and long term Care to residents within NPT Borough.

 

All applicants need to be able to work as an effective team member of a multi-disciplinary, multi-agency team in co-ordinating the above.  To work as a member of the team helping to deliver a short/long term programme of care/therapy to clients who have become unwell or lost their skills and confidence for living independently. To provide a high standard of personal care and support to people within their own home. To take direction from supervisors/therapists to implement planned programmes of therapy/care from support plans. Ensure the delivery of a high quality service in line with Registration and Inspections Social Care (Wales) Act 2016, statutory duties, policies, and procedures.

 

To undertake this challenging role, applicants must have experience of working with individuals in need of care or support in a health and social care setting, must have an understanding of supporting people living with dementia, Successful applicants must register as a Domiciliary Care Worker with Social Care Wales. (Registration and Inspection of Social Care (Wales) Act 2016.)

 

You must have QCF qualification Level 2 in Care or be willing to work towards the qualification within a specified period of time, hold a full valid driving licence and have access to a vehicle insured for Class 1 business use.  It is essential that the applicant can travel extensively within the NPT authority.

The Community Resource Team supports individuals to live independently within their home environment. The Team comprises of the following: Early Response, Mobile Response, Better at Home, Telecare, Reablement, Bridgestart, Community Independence and Wellbeing Team, Community Occupational Therapy and Integrated Community Equipment Service.

You will be a key part of the CRT Services providing Domiciliary & Therapy support across Reablement, Early Response, Mobile Response Bridgestart, and Better at Home. This service aims to support people to continue to live in their chosen environment by preventing avoidable hospital or care home admission and to facilitate early discharge from hospital, sometimes on a day to day basis. You will be expected to provide enabling support for a number of people who have lost their skills for a variety of different reasons. Providing encouragement, promoting independence and building confidence. You also provide support with personal care and meal preparation as required across the Bridgend Area.

The service (except Mobile Response) currently operates daily between 7:30 am to 10:00 pm and staff are required to work between these hours to meet service users’ needs. You must be committed, enthusiastic and flexible. Full driving licence and access to own transport is essential. Mobile response provides 24 hour cover’.
